APPOINTMENT CANCELLATIONS, NO-SHOWS and LATE ARRIVALS
Life happens–we get that. If you need to reschedule an appointment, please inform our office at least 24 hours in advance. Cancellations in less than 24 hours and no-shows may result in a charge for the missed session.

If a client arrives more than 15 minutes late, the appointment may need to be rescheduled, or they may have to complete the session with less time.

PAYMENT
Our office accepts many forms of payment, including credit cards (may be subject to fees), Zelle, and personal checks.
Invoices are sent out monthly. Services covered by insurance will be billed directly to your plan. Please note that unpaid monthly balances may incur a late fee.

BILLING QUESTIONS OR ISSUES
If we find a problem with billing or your form of payment, we will do our best to notify you. Should you encounter questions on concerns, please contact us and our administrative staff will get back to you within 24-48 hours.

CONFIDENTIALITY
We take our clients and families’ privacy seriously. While MBC staff follows all HIPAA guidelines, confidentiality may be disrupted if a client poses a danger to themselves or others, reports abuse, or if required by law.

MBC is in-network with most major insurance companies. Our administrative staff is here to help you navigate insurance authorization and the submission process, as well as billing to in-network providers.
To find out if we accept your insurance, please complete the form below and our office will respond in 2-3 business days.

Please note this form is not to arrange an intake call, schedule services, or be placed on the waitlist. For assistance with these options, please contact us.
